OPEN WORK PERMIT

So, I'm trying to find the open work permit form to fill out . According to this : https://www.cic.gc.ca/english/helpcentre/answer.asp?qnum=1163&top=17

it says :
If the sponsorship application hasn’t been submitted yet:

• Put the sponsorship, permanent residence and open work permit applications into one envelope.
• Make sure to include:
  • all supporting documents for all applications, and
  • proof of payment, for all applications.
• Send the envelope to the Case Processing Centre in Mississauga (CPC-M), Ontario.

But as I go through the process the find the application, it says that I have to apply online, but how can I put the open work permit in the envelope if I apply online.

Does anyone have the link to the form?

Hi, I am going to apply under the SCLPC class (spouse) and also was looking for the open work permit application. As I understood from a youtube video, I can download and fill in the IMM 5710 form and attach it to the sponsorship and PR applications. This form is Application to Change Conditions, Extend my Stay or Remain in Canada as a Worker. I am not 100% sure yet, but I believe this is the one we can attach to our sponsorship application.
