Open work permit please help

Where it asks for the job ill be doing and where ill be working...do I just leave it blank?

I've filled in the application as best I could and then attached an additional page explaining my answers. I put N/A in some mandatory columns since it's a general app an you won't have a prospective employer with an OWP. So do your best, explain the gaps on a separate page.
