Open Work Permit for the spouse who is in Canada of a full-time internal student

Hi, PPM.
Sorry to bother you.
I am an full-time international student.(My study permit will expire on July 2012) . My husband and my daughter have got their visas and are visiting me in Canada now.
I want to apply for my husband the Open Work Permit. About filling the forms, I have a several questions:

For IMM1249
1) In what capacity should we fill in the form? In my husband's or in mine?
2) In section D-“My request”,
part 10: should we fill it in the capacity of mine or my husband?
part 11: to support my self in Canada, if I fill in with a number, should I provide a bank statement to prove that?

For IMM5409
Since we are married already, do we have to submit the IMM5409E(statutory declaration of common-law union) ?

If we want to apply online, how can we sign those forms? We sign them and then scanned them and upload them? It is confusing.

For my daughter who is 7 and is attending primary school in Canada.
We didn't apply study permit for her. So is it illegal for her to study in Canada?

I really appreciate if you could answer my questions.

1. Your husband's as he is the one that wants the open work permit.
2. Open work permit.
3. Your husband to the same date as your study permit and don't forget to include the child.
4. Yes provide a bank statement.
5. No, you provide proof of marriage.
6. It depends on the school board, some insist on seeing a study permit, check with the school board where the child will be attending school. If they require a study permit, then apply for 1 at the same time.
7. Yes it has to be signed.

Two more questions to ask:
If I apply work permit for my husband and study permit for my daughter (that will require two costs respectively), I know my husband will get the same expire date for his open work permit as my study permit. But
1) will my daughter get the same expire date for her study permit as mine?
2) will the officer issue the new visa for them which would have the same expire date as my study permit too?

Yes, and yes. Although it maybe shorter if your child and spouse require medicals, depending on their country of origin.

Hi, PPM!
My husband and daughter came to Canada to visit me with temporary resident visa which will expire in September 2011.
Now, my husband has got his open work and my daughter has got her study permit, both of which will expire July 2012.
They are required to take a medical exam.
But we have not received any newly-issued visa.
My question is do they still need to apply for visa extension?
