Hi,

My wife is planning to apply for open work permit soon as I have already got Study Permit.
Are following forms correct to apply for open work permit?

1295E
5257B_Schedule 1
5645E

Also, would like to know what are the documents needed to apply for the same as the checklist is bit confusing.

Thanks,
Vish

I applied for an OWP along with my wife's student visa and most of the forms you've mentioned are correct. I had to fill out another form 5707E. As for the documents, these are some of the docs I submitted along:

1)Purpose of Travel document: I uploaded my marriage licence

2) Letter of explanation: Consolidated letter to the Visa officer where I presented my case (wrote an SOP) that my wife needed emotional and financial support and thus I had to tag along. The letter to visa officer was below 4mb and contained the following details :
- a table of content which would tell the visa officer all the details I had added to the letter
- My bank accounts disclosing all the money I had (4 Months account statement)
- Affidavit by my dad disclosing that I was the soul heir to his properties. This would show the visa officer that I had a connection back home.
- All my investments including mutual funds, Fixed Deposits etc.
- Salary slips for the last 4 months
- A small table of content which disclosed my employment History
- My experience certificates from my older companies
- Finally, my educational certificates.

At the end of the day, I took a chance where I put all the information I could gather to let the visa officer decide my fate. Although the main applicant was my wife but I still felt I had to write a letter to the visa officer describing our situation from a spouse's end. I'd read a lot that people should put only adequate documents but I didn't quite agree with that. I was confident that putting all the documents would make our case stronger and in the end we both did get our student/owp visa.

Hope this information helps.
