Open work permit applying at the same time as live-in caregiver PR?

My fiancee and I will be applying for PR and OP in may through the Live-in Caregiver pathway. We have all the required documents for the PR, but we are looking to get the required Docs for OP to apply at the same time.

Looks like we have to fill out the form IMM5710E. What I am confused about is there is no mention of "Open Permit" anywhere on the forms.

Can someone let me know what their experience is with applying for this?

Yes, fill the form IMM5710 ,check initial work permit/work permit with a new employer.
Page 3 of 5 of the formetail of intended work in canada,leave question 2-6 blank.
After completion of your forms on your bar code page which should be placed on top,write open work permit on a sticker note and paste it on you bar code page.
