Open Work Permit Application (IMM4509E question!)

Good Evening! I am currently filling in and collecting all the forms needed to start the Sponsorship Application. I am staying in Canada right now on a Visitor's Visa and plan on attaching an Open Work Permit application with my Sponsorship Application. According to CIC this would allow me to be in an Implied Status even after my Visitor's Visa expires, making sure I won't get deported or anything. Plus I would be able to get back to work.

To get to the point, my Sponsor is my Common-law partner who is a Canadian Citizen and lives in Toronto. In the Open Work Permit application package there is a form called IMM5409E, known as the Statutory Declaration of Common-law Union. according to the introduction page it says this is only needed to be filled in if my Common-law partner is accompanying me to Canada..

Now since she is already a Canadian citizen and I have already filled in much information about my Common-Law in the actual Sponsorship Application, do I have to fill in this form as well? It's asking me if I have life insurance with her as the beneficiary, but I am not even a Canadian citizen yet!

You should be asking your questions in the Family Class forum. Yes, complete IMM5409E for a common-law sponsorship. If you don't have life insurance then answer NO to that question.
