OOPNP PT Notification

Hi,

I just received PT Notification. I am applying as a married person. I am confused with some questions on page 1:

• Have you, your spouse or common-law partner, or any of your dependent children, already submitted or been included in an Application for
  Permanent Residence to Citizenship and Immigration Canada (CIC)?
• Have you, your spouse or common-law partner, or any of your dependent children, already submitted or been included in an Application under
  a Provincial Nominee Program to another province or territory?
• What would be the fees i would be paying?
