OOPNP application form question

Hi everyone,

-When I filled all the required fields of the application form, in the last page, there are 6 consecutive empty fields showing up for signatures. Should I really sign all of them?

And

-After I print the application form, should I put N/A on everything that is irrelevant? Even if I could fill them out electronically because they were irrelevant? Let me give you an example. I am not in a common-law partnership. Do I need to put N/A on all of those fields related to common-law partner after I print the application?
