Ontario Photo ID Card

Hi,

I have few questions about Ontario photo ID Card and Health Card. I became a landing PR on 7th Oct 2015, then i had to return to USA to complete my contract. I recevied my PR card in June 2016. My Wife's Indian Passport had me listed as her husband and changed her last name while she was in India 3 years back. Since then she has been living with me in USA. However in her passport her, first and last name was in the last name column. We applied our PR card with that pasport and now her PR card & COPR also has her first & last name in the last name field. To get all this cleared, this year, we got this name split done on her Indian passport to get her first name and last name in the correct fields. But i guess this got me in another trouble..So my question is:

- I kow we have to get our PR card re-issued with correct name, but can i apply for her Health card and Photo ID card with new passport and current PR card as identity proof. She is 3 months Pregnant and we need to have this health card / photo id card before she is due

Please advise urgently, i need to get all this sorted as soon as possible...

it's important to remember, if you are not currently LIVING in ontario and you attempt to use OHIP, you will be committing fraud and are subject to having to pay back for services. your post does not clarify if you are currently living in Ontario. The change of information form requires you to put your current address, which must be in ontario to qualify for continuance of OHIP. You may be required to submit original documentation to prove current ontario residency. check the list to be ready to present the documents if they are required:

I am currently in Missisauga. So on that front, i should be ok, plus my wife will always be in ontario..even if i have to travel for work purpose.

But my question really was that mis-match in name between PR card and Passport, (though it is really not a mis-match but just they are in right fields in passport now and PR still has to be corrected) will that cause an issue for me to get a Ontario Photo ID card or OHIP card.

you need to read the rules on the website. you may be able to get away with providing a marriage certificate to show the name change.
