[ONIP] Any histories on being in DIP and rejected?

Hi,
I applied through Human Capital Stream (Express Entry Human Capital Priorities).

I've read that DIP is considered 'safe'
but have there been cases where it gets rejected after staying in DIP for a while?

Submitted June 12th
Assessment July something
EE expired, updated, and re-connected August 13th
DIP sometime btw August 20~Sept 1
