On J-1 visa in USA got medical exam forms

Hi Guys I am from Egypt, live in USA on j-1 visa doing medical fellowship. I got the med exam forms from Buffalo, I still have 2 years in USA to finish my training, what should I do?
1- Should I delay my med exam as much as I can in order to prolong the process?
2- Should I do the med exam, get the visa go for landing then come back to USA?
3- My J-1 visa stamp has already expired but I am on a legal extension status, will I have problems comming back to USA within 1 month?
4- Can I live in both countries at border areas where I can go back and forth as my job in USA will pay me better?
I hope to hear answers from those experienced with such a situation

i dont think u can delay medicals more -it might lead to some problems
After medicals when u get permanent visa - u have one year to land
so u can land after 1 year of yr medicals to get more time
1. If u r doubtful where to live, in usa or canada, its advisable to get PR for canada
after u land u can come back to US for upto 3 years and still retain yr status if u move back after 3 years - the idea being to figure out where u wanna stay in (1 + 3 = 4 years) after u get yr medicals done
2. If u think right now --- u will be paid better in US and u will get PR in USA ----- u should save money by not paying RPRF fees 490 CAD or getting it refunded if u have paid it by not landing
3. I dont know if u live at border - whether u can retain yr residency in canada or not, past 3 years
4. Better to choose one - then both
5. I dont know home country residency requirements with J1 visa, if they apply then u should think about residency in egypt too cause its still required even if u r PR of canada

I am on H1 visa now but not stamped on passport
I have an expired F1 visa and didnt have any problems coming back to USA last time I went there
I will be moving permananetly to canada when I get a job there
I have spent 3 months in US after getting canadian PR and the canadian officer was questioning me about residency requirement when I drove to canada
r they really serious of the requirement????
