I got a CoPR a few days ago, and I have been in Canada since 2010, and I have valid Driver's License.
"Establish residence" means LANDING date with CoPR? or just residence address and date from the Driver's License?

and Is it true that I have to wait for 3 months to apply for OHIP and then wait for 3 months again to receive OHIP Card?
 
You can apply now and show your landing papers. If they are satisfied that you have proven that you are living in Ontario, you will most likely get OHIP coverage 3 months after your landing date.
