Offshore EE profile applicant now inland. Changes to make in EE profile?

My spouse and I made our EE profile in April 2016 and we have a score of 473 as offshore applicants.

He is now in Canada and will be starting post grad in September while I'm in Pakistan and my SOWP is in process. What changes should we make to our EE profile? The NOC for his post grad is different from the NOC of his foreign work experience as he is changing careers.

Please reply, we are really confused. ???

You shouldn't have to make any changes.

If my spouse doesn't declare his inland status, how will his medical be processed when/if we receive an ITA?

CIC issues a letter after the ITA for the medical to be done (I'm not sure if the medical request is standard and applicable to wherever the applicant is residing or not). He is the Principal Applicant and If he doesn't mention in the EE profile that country of residence is now Canada, will that request for medical be good for getting an appointment with a physician in Canada?
