Oath Ceremony - Where can I find the Personal Release and Consent form?

Recently received my final notice to attend an in-person oath ceremony. The document states I need to bring a signed copy of the Personal Release and Consent form, but I can't find the form within the pdf. What is the form and how can I get it? Thank you.

It should be at the end of the PDF file (the notice you received)

If it's not there, you can reply to the email and request them to send the form.

They usually also have copies of the form at the IRCC office or location where the ceremony is going to take place.
