Hi guys, can you tell me if the following duties are okay for NOC 33102

  • Provides care coordination for clients, including: Conducting InterRAI Community Health Assessments (CHA), collaborating with clients and caregivers to prepare services plans and associated service agreements, facilitating the development of Coordinated Care Plans with high risk clients in collaboration with clients, caregivers and internal and external partners, monitoring and evaluating client outcomes, coordinating and following up on referrals, documenting all activities as per unit/program policies, enrolling high risk clients in the Community Agency Notification Program and identifying the need for and participating in or facilitating case conference and clinical meetings.
  • Assist clients with menu selection, supervise their daily routines including setting up exercise routines, leisure activities, accompanying them on recreational activities and supervise personal support workers assigned to the client.
  • Monitor client outcomes and changes in health and report client’s fluid and meal intake based on the information provided by the personal support worker on AlayaCare.
  • Observe and monitor client’s status and document client care on AlayaCare and Inter RAI CHA assessment.
  • Administer, in emergency situations, first aid to injured or ill clients and report to paramedics on arrival.
  • May require transporting clients in wheelchair to medical appointments.
  • Responsible for maintenance tasks such as assisting with the set-up and maintenance of Hoyer lift, wheel chair,
    grab bars, bath bench and raised toilet seats for the clients.
  • Maintain inventory of supplies
  • Conducts human resource activities to ensure sufficient staffing levels, including scheduling, identifying recruitment needs, screening, interviewing, hiring, orienting, supervising, evaluating, payroll and addressing work performance issues in compliance with WoodGreen polices and Collective Agreement.
  • Responds to client issues in a professional manner by using problem solving, conflict resolution and de-escalation techniques; makes referrals to The Toronto Senior’s Helpline or Social Work in accordance with unit/program procedures.
  • Mediates conflicts or issues involving clients, PSWs, and other service providers in consultation with the team and Manager as required.
  • Liaises, consults and collaborates with members of other program area staff and other service providers in a professional manner.
  • Collects statistical data using relevant databases (i.e. CIMS, QHR), as required for the accountability of the program.
  • May perform other duties such as making beds, maintain clients room, cleaning of Hoyer lift, wheel chair, grab bars, bath bench and raised toilet seats for the clients.
  • Delegate PSWs with safe lift and transfer, medication management and compression stockings.
  • Participates in operational, quality improvement and risk management planning and monitoring processes.
