No surname in degree certificate

On my passport and national identity document, my name is written as <FIRST NAME> <MIDDLE NAME> <SURNAME>
In my degree certificate, my name is written as <FIRST NAME> <MIDDLE NAME> because it must follow my birth certificate and it only contains my given name, which is <FIRST NAME> <MIDDLE NAME> although my father's name is written with surname on it ( <FATHER'S FIRST NAME><FATHER'S MIDDLE NAME> <SURNAME>).

To deal with this problem:
1. Should I fill the "Other names" field on the application form?
2. Should I make a SAME NAME affidavit or just explain this on my LOE?

Please help.

Anyone here ever had similar issue? Or do you think it shouldn't be an issue as long as my father's name contain our surname and that the surname is also written on my passport and national identity document?
