No name in Birthcertificate

Dear all,

My hubby's birth certicate did not have a name. It had his DOB and motherand father's name. I have attached a copy of 10th marksheet. he is a dependant. Is it OK?

Is there a format / content specified for what needs to be on the affidavit?
