No barcode on ANNEX.A

Hi all,

I receive the email a couple weeks ago and managed to print the Annex. A form from the email. Some people are saying there should be a barcode on the Annex. A and here is what I have in my email:
Barcode merge field was not found in header record of data source.

Does anyone have the same issue?
