new member needs help!

hello,

so this is our scenario, my husband and I are temporary resident in canada until june 2013.my husband got extended in his work until jan 2014.his office helped him with renewal of his work permit and with my application of work permit. we received it just this week. how do we renew our Temporary resident visa, me as his dependent? we're not sure what application form to fill up. Will there be a need for a medical? we would want to go out of the country for a tour on dec that's why we wanted to apply asap.
