New IMM5669E

Hello Everyone!

So quick question about the IMM5669E form.

CIC has given us this form to complete again and the request for the medical and police report (as they both expired). Now my concern is that when I check the version of the form they provided with their link, it's an old form, from 2012.

However, if one were to check the CIC website and look at the application forms, there is a new IMM 5669E form which has a validate button (similar to the IMM 1344 form).

So my question is, which version of the form should I submit to them? Is using the 2012 version okay? (After all, that is what was in their link) or should we be proactive and use the 2016 form.

If they asked you to complete a specific form with a specific version, I would follow their direction.

I would use the form they sent to you because your initial Application is from 2012 and not 2016.

Also, if you want to be 100% sure, contact the their office and ask for specific instructions because you dont want anything to ever come back once you send it in.

Actually my wife's application is from 2015. It's just that at the time, that form was not available, so we were using many of the 2012 forms.

Now that it's 2017, the new forms are out so I was just unsure of which form to submit.

Honestly speaking, I would personally call cic and ask them which one to use just to avoid the form being returned. Also, if most of the forms were 2012 form and they form they sent was 2012 I would submit the 2012 form to keep it consistent
