NEW FORM OR EXTRA WHITE PAPER

Hello all

if we need to give some more detailed information then whats suitable?

1: Should attach a plain simple white paper and write form name and question paper

or

2: Should download the same complete form again and just fill the required field only

3: Should download and attach the only page of form,on which field is required to complete.
