Hi - I live in the US. I applied back in 2010 for Canada Permanent residency through the Skilled Worker and I was granted it and I was supposed to Land in Canada by March 31st 2011. However, my Green Card was also granted in the US and I decided to stay here and I never landed in Canada. If I want to Immigrate to Canada now, Do I need to re-apply or is there a way I can actually start off with my old application? Can I request them to extend or renew my old application?

Thank you!

I am afraid you may be asked to re-apply if you are in one of the 50 eligible NOCs listed for 2014. IELTS is compulsory even if you are a Work permit holder or graduate of Canadian PhD. Age, Education, Work Experience, Adaptability( relations in Canada and Your spouse's IELTS If need be),Offer of employment in Canada are the major factors for consideration which you must have 67% rating in order to be eligible. What is your NOC?

You need to reapply. No - you cannot request that CIC extend or renew your old application.

The visa you were granted back in 2011 had a "use it or lose it" policy attached. Since you failed to "land" in Canada and become a permanent resident before the expiry date on that visa, it's dead and long gone.

You have to start the process again from scratch.

Accountant! I still have all the Landing papers!

And it is probably more complex and expensive as it was back in 2011, correct?

Yes - it's more complex and expensive.

Taking the IELTS test is now mandatory - as well as getting your education formally assessed.

Unfortunately your old landing papers are useless. You'll literally be applying again from scratch and will have to meet the current requirements of Canada's immigration programs.
