Never applied for visitor visa - but got a refused survey mail to my email id.

Hi, I received the below survey mail about a visitor visa application that I never applied. I am a PR since late 2018, and I have never applied for a visitor visa for myself or family members.

We would like to get your thoughts on your recently refused Temporary Resident Visa (Visitor Visa) application and the explanation provided in the Refusal Letter.
This survey should only take approximately 10 minutes to complete.

Is this a glitch or spam, or should I contact customer care? Please suggest.
After receiving this mail, I even tried giving my email id to check if any username is associated with the email id, but I did not get any mail.

Thanks for the response. But if my email id is associated with the application, I would have received emails about the application, and this is the first time I received an email. Maybe a system glitch.
