Need urgent help with filling IMM5406

Hi all,

I received an email from ircc asking to submit the IMM5406 document for additional family information.

I have 3 queries on how to do this:

1. Is it okay to print the form, fill it and scan it and send it back to ircc?

2.There is hardly any space to fill the full address for each member in my family. The form says to attach a separate sheet if additional space is required. But do I attach the same form with the remaining information written?

3. My parents are divorced and I have no information about the whereabouts of my father so I thought of leaving the father field as not applicable, but do I need to justify that? If so how do I do that?

If anyone has filled out this form, please please help me.

1. Yes, you can print it.

2. Entering the additional info in a Word document is fine.

3. You can't leave your father blank. Put everything that you know and explain on your additional sheet that you have no contact and know nothing else.
