Need to confirm CLB report card.

Hello friends, i took CLB test in 2014 (ON) and got overall 8. I received a hand written fold report card . My name, phone number and client id number was mentioned on the first page and in another page score, comments, accessor's name & date mentioned. At the first page, under client profile, "This assessment is for the purpose of placement only in a language training program" is written. Is that the report card i can submit with my application? Or do I need to collect something else. I only received that card. Please confirm. Thank you.

For IELTS, as we submitted original to CIC during immigration, if we now submit a copy, will that be accepted at the final stage when they ask for all originals?
