Need template for Marriage affidavit parents PGP 2024

Hello All,
My parents don’t have marriage certificate. I read somewhere that we can provide a marriage affidavit instead but I cannot find a template. I would appreciate if anyone else has used a template for a marriage affidavit that they can share.
Also do I need to get this notarised? On a stamp paper?
