Need some clarifications regarding documents required and forms to be completed.

I have a few questions regarding the documents required and information in the forms to be completed for the CEC

1. Checklist requires a legal document in support of change of name. I had my surname added to my given name in the Indian passport. Is the notrized copy of amendment page (where surname is added on the passport) a proof enough or do i need to provide any additional legal document in support?

2. On my passport, my parents' name does not include their surname and it's not there on their passports either. My question is, on the background declaration and additional family information forms, what do I fill in the surname/family name section for my parents? Do I leave it emply or fill in our family name even though its not added on their passports or mine?

3. Do I need to provide copies of my educational certificates (diploma/degree) even though its not requested in the document checklist? A few people in my circle have me confused over this.

Any bit of information regarding any of the above is greatly appreciated.

However, the answer is no to this question. You only need to provide what is requested.

I had the same situation. I added the passport page where they have written that "The holder of this passport was previously known as bla bla"
Also, I added a page of Maharashtra Government (provincial in other words) proof that I changed my name.

In my opinion, there shouldn't be a problem if you add the family name as long as you can prove it that its true (if they ask to). I'd say consider more opinions...

I agree with jsm0085 that u don't need to give these certificates.

- Is your surname changed or added or misspelled in your passport?? You only have to submit a legal document if you have actually changed your surname. Or amendment page is more than enough. If you are not convinced include an affidavit.

- Just enter his full name according to his passport which I believe from above is same in your passport to you, then no worries

- Not required. Its not in the check list you do not need to provide.

@ siroccoon: I had my surname added to passport as previously it was only my first name on it.

I talked to a CIC representative and he told me to include my parents surname in the forms even though its not there on my passport or theirs. He suggested including a letter stating that even though i have entered their surname in the forms its actually not there in their passports or mine.
