You must do the following when submitting a paper version of the form with your initial application:
In the signature section, type the name of the person for whom the form is being filled out. Also enter the date that the form is being filled out.
Validate the form.
Print the form.
Sign your name by hand. (Note: For principal applicants under the age of 18, a parent or legal guardian must sign the form on the dependent child’s behalf.)
Important: If the form is not signed and dated per these instructions, it will not be accepted and your applications will be returned to you without being processed.
