Need help to restore my status!help please

Hi guys,
I have applied for study permit extension, then I got refused due to answering a wrong question, I have answered "yes" in the question asking if I am applying for work permit along with my study permit extension. So, now I am applying for restoration application to be able to extend my study permit.
I am confused about two questions in the background section in the application:
First question is:
-Have you ever refuseda visa or permit,denied entry or ordered to leave canada or any other country or territory?
What should I answer "yes or no".
I got refusal when I applied for study permit extension.

The second question:
- have you previously applied to enter or remian in Canada? Yes or no? Is that a "yes" ?


Please help!!!! I have less than 90 days to sumbit my application and I dont want to make another mistake.

Anyny opinion will definitely help me. Thank you.

Yes to both. Include some short explanation/dates along with any doc numbers such as application number etc in explanation.
For the second question include your previous study permit details/dates and any extensions or work permit applications.
Also always talk to your schools international office about correct way to fill out forms and required docs before submitting anything to IRCC.
