need help to fill declaration of commom-law union

please i need help to fill declaration of commom-law union 5409e if some one did recently please help to do i have couple of questions.

we are together since 15 years with 3 children

1-jointly signed a residential lease -NO
2-Jointly own property -NO
3-Have Joint Bank Account -NO
4-Insurance on my self which name my common law as beneficiary -NO
My Question is what other Documents i can provide to prove them ?

What do you have proving that you have lived together for 1 year for example the last year?

We have 3 children
All of them under my sponsorship in country of residence where we are living
What else I need to give them to prove?
