Need Help submitting case specific inquiry CSE

Hi all, i'd like to ask for some help / advice.
Today I received an email from CIC requesting an additional document (police certificate from the Netherlands) with regards to our spousal sponsorship application. However we did not provide it with the application because it says very clearly that a police certificate is only needed if the person resided in the country for more than 6 months, and my wife was there for only 1 month.

So with that in mind I'm going to submit a CSE (case specific inquiry), though i don't know whose name to input under "applicant's name" would it be me (the sponsor) or my wife?

You can use any name yours or your wife doesn't make any difference. Make sure you write down the file no.
