Need help po,.. Newbie..

Hi guys, I'm newbie here,..I just Got My LMO last Feb.14,2013 and my employer send me a link: a form of application for work permit and my Family Information But I'm not done yet because i have a job and i don't have enough time to gather some documents,. my question in my family info,.do i have to include my previous marriage even,. were not legally separated and we had our own family?how about my live in partner and our daughter? what if i choose single? And What other documents po ba need i attach?
