Need help in Spouse open work permit application filling

Hello All
I am in canada on PGWP, I have applied for my wife for open work permit 5 months ago but her visa has been refused, CIC mentioned that my JOB in canada doesnt fall under NOC O,A AND B category.

Now i am working as business analyst which comes under NOC A category and would like to reapply for my wife now

I found below application package and planning to apply on paper.

IMM5488 Checklist
IMM1295 WORK PERMIT
IMM5257 TRV APPLICATION
IMM5707 FAMILY INFORMATION

AND SUPPORTING DOCUMENTS
Marriage certificate
Bank statements
My wife education and work documents
My work permit,passport and pay stubs

I am confused in filling IMM1295 Under Background info section 2b and 2c
2a)have you ever been refused a visa or permit,denied entry or ordered to leave canada or any other country?
2b)have you previously applied to enter or remain in canada?

My question is since my wife visa rejected before,so do i have to tick yes for the both questions or not.

filling TRV application form IMM5257
when filling IMM5257, she has to select the principal applicant or The spouse,common law partner or dependent child aged 18 or older of the principal applicant?

She is the only person applying for work permit and TRV and we dont have any children, and i am not applying since i am already in canada. so does she become the principal applicant or not?

And finally
When we applied for work permit first time we sent the original passport also along with application, so do you suggest me to include original passport again with the application?

Please answer my questions and help me in finding a solution, I dont want to take chance as i want to stay with my wife and make career.

the answer to this is definitely yes. And I am not sure about the other questions and don't want to mislead you with my assumptions, so you better wait till someone else comments.
