Need Help in filling IMM 5669

Hi,

I have few questions in this form can anyone answer below questions

1) personal history for me, spouse and child. I am living in dubai and after marriage my wife stayed for 6 months with me in dubai and later she travelled to india for delivery and stayed for an year and came back to dubai recently. Do i need to include this time in the form or just can write for her first visit to dubai to second visit to dubai she stayed with me like 2012 to 2014

2) And for the address i was living in bachelore accomidation while i was studying i dont remember the home owner address can i just write the streetname and city name

3) My wife is not working and she is staying as dependent what i have to write for status spouse visa or dependent
4) What should be the status in india i wrote citizen is it fine

Hi Chandar,

To be on the safer side you have to denote all the actions from which month till which month your wife was with you etc.

You should first write the period she was with you. then the period she went for delivery and again the period when she joined. Do not miss any information.

For india, Citizen is ok

Thanks, what to do for the institute name and city name if it is not fitting in the given boxes? should i print it on a sperate white paper?

Yes please write the form name and number make a similar table as in the application form. Fill it date and sign it (mandatory)
