Need help! For spouse work permit

Hello guys! I need a help and want to ask. I applied my permanent residence in Sept 2016. Then I went to back home for marriage. I added my spouse file with my file. Now I wanted to apply for spouse Work permit. Would anybody help me how can I apply. What will be the forms name and the other options. I will appreciate your help. Thank you

I applied in the Provincial nominee program. And I am the applicant and my wife is dependent. She is now added in my file.
