Need help for fill-up background form_ Need urgent support

Hi,

I applied Canadian student visa in 2000 but I was refused. I could remember the reasons of my rejection. What should I write in my application?
Can I write the below line or need more explanation.
"I applied Canadian student visa in 2000 but I was refused."

NO.. you should Attach the Refusal letter which you receive at the time of refusal and your reference file number of the last attempt.

Thanks for advice. It is already passed 14 years and embassy did not provide refusal letter and application file number. Then what should I write?
