Need help filling out "Other parent" section of citizenship form

My children were born in Thailand via surrogacy and were issued Canadian citizenship certificates at the embassy there. We recently moved back to Canada and are applying for new certificates as their last names have recently been changed. I'm trying to complete CIT 0001.

Do I still need to complete the section for the "other parent" (the Thai surrogate)? I do have a waiver that the surrogate signed to waive their parental rights and we submitted that along with our application and left the "Other parent" part blank. Will I need to submit that waiver again with my application?

I wouldn't suggest leaving any section blank. Write "see attached waiver of parental rights," and attach a copy of the waiver. It may not be necessary to send it again, but it's simpler to just attach it now rather than finding out later that it's required. Also, send the original certificates of citizenship and a certified copy of the change of name document.
