Need advice for husband & wife applying from same countries; but dependent children from different country.

Hello folks

Can I get some information as well as suggestions from the expert or experienced persons regarding my issue, please?
I am planning to apply for student visa from Japan; my wife as a dependent. Both of us are staying here in Japan. But our daughter (9yrs) is staying in India with grandparents. I am gonna add her name as an accompanied person. In that case
1> Does she need to apply separately from India or its okay to apply online with me?
2> If okay, after confirming does she need to present her passport in Canadian high commission in India or Here in Japan for visa stamp?
3> Is it okay to do medical alone in India for her?
Just to let you know that after getting visa my wife will go to India and then wife-daughter with move to Canada together.
Is there any information that will be information in this case?

3. Sure. Note that medicals aren't really happening right now due to COVID-19. so there will likely be a delay.

Biometrics can be done anywhere you want. You aren't assigned an appointment at a specific location.
