Hello
My canadian wife has two names listed on her passport (Sarah Elizabeth for example). Her second name though is considered a middle name and on most of the forms there is no space for middle names. Although on her passport it shows Sarah Elizabeth under name. Should we always use the two names on the form every time it asks for name?
 
Yes, if its on her passport it should go on the form.

If I'm not mistaken (don't have the form in front of me) it says "given name(s)". Meaning you could put more than one..meaning you can put her middle name in there.
 
Yes, like Weegie said, all the forms say “Given Name(s)” and you should always be listing first and middle there together. Because not everyone has a middle name from all countries/cultures, I would imagine is why they lay out the forms that way, instead of giving middle name its own field.
