Name on Uni Application Different From Passport

Hello,

I am in the process of applying for a study permit. I have already been accepted for a master's program at Simon Fraser University this coming Fall.

My full name includes two last names (surnames), in the university application I only included my first last name because it made no indication of requiring the second one and for all applications, I have the habit of only use the first surname unless otherwise required.

The study permit application requires all my surnames as indicated in my passport. I fear this may cause a conflict when they compare my study permit application with the offer of admission document and information provided by the university.

Study Permit Name: F M S1 S2
University Profile: F M S1

In the study permit application should I include my name as indicated in the university application under the "Have you ever used any other names (e.g. Nickname, maiden name, alias, etc.)?" or not?
