My wife has changed her name on passport (i.e. last name).

And her birth certificate and all other certificates and marksheet has her old name (i.e. her old Last name). Is that ok if I provide these documents as it is or I need to do something else?

And also in immm0008_1e, there is a column that asked about other name you have been using. Should I provide the name which was she using before marriage.

Her birth certificate will always be in the name she was given at birth - it does not get changed just because she marries. So, no, you don't do anything with the birth certificate.

Yes, in IMM0008 note her name before she was married under "other names" she has used.

Can you tell me where ever there is space in column (in all the form), should I write Not Applicable or should I just leave it blank.
