Name Misspelled in Educational and Employment Documents

Hi,

I have submitted my PR application on 20th Oct 2017, but the name in my educational and employment documents are misspelled, so I wanted to submit a Letter of explanation with an affidavit or notary.

I am looking for a sample format of LOE and Affidavit, I will be thankful if someone could assist me with it.

It could be like this:

Dear Sir or Madam,
I am xxx, my application number is xxxx submitted on xxx. My application number is xxxxx and my UCI number (if you have) is xxxx.
After submitting the application, I have noticed that my name is misspelled in my educational and employment documents.
I have already taken steps to have them corrected (provide proof that you have actually initiated steps for doing so) and hereby declare this by an affidavit as well.
