Name --> Has this person ever used any other name?

I have a query on this.

I normally am known just by my FIRST NAME (Sandy) and LAST NAME (Gajjar)

However in two places:
a) one of my old job from where I am taking reference letter, they mentioned my name as FIRST NAME + FATHER FIRST NAME INITIAL + LAST NAME -- which goes like Sandy K. Gajjar

b) Also in my UAE work permit which is stamped on my passport, my name is mentioned as

Sandy Gajjar Brij Kishooore Gajjar

Please note that they misspelt my father's name on the UAE Work Permit - however the emirates ID mentions the same correctly.

My query is -- should I mention these names in the General Field of "HAS THIS PERSON EVER USED ANY OTHER NAME?"

I have already got all my PCC - and I have not mentioned any other name in the same.

Just get an affidavit of one and the same name (confirmation of name) for all the variations & spellings of your name.
