Hi,
I did search the forum for similar posts but my situation seems unique...i have my passport in my maiden name, didnt feel the need to get it changed after marriage, so at the time of applying for ielts as only passport is accepted as proof, i had to give my ielts also in my maiden name, educational credentials also have the maiden name.

but my bank accounts have my married name, also national identification document other than pan card has my married name, i have a marriage certificate and my daughter's birth certificate also has my married name.

now, while applying to OOPNP i attached my marriage certificate and a notarized/stamp duty paid self declaration that these two names are mine and valid.

i wanted your opinion as to whether there is anything more that i can do to clarify and avoid confusion when applying for PR (hoping i get an ITA in the next round sitting at 459)

any advice will be greatly appreciated

You've already done the best you could (notarized self declaration) short of legally changing your name. You could consider doing that depending on how long it takes in your part of the World to do such a thing. If it doesn't take too long, that's the best way to avoid any further confusion.

All the best!

Well, it will raise eyebrows. That's what officers are trained for - to notice discrepancies and ensure that there is no attempt at fraud. In your case though, that notarized declaration will help.

This situation is similar to mine and I am getting nervous now.
I have passport, bank accounts, work ex, educational credentials, PAN all in my maiden name but on my marriage certificate and my kids birth certificates, voters ID card I have married name. My husbands passport and my kids passports too have my maiden name as I preferred not to change. I never thought of attaching a notarized affidavit until I came across this thread.

Should I be bothered that my case might raise an doubt?

Don't be too worried if you haven't sent it. If they identify a discrepancy, they will check the application in tot or they might ask you to clarify or cancel your application.

Even in the unlikely scenario of cancellation, you can send in a CSE and get it reopened in weeks.
