Name Difference

Hi,

I am planning to apply for immigration. My education documents and passport has name difference.

Education Certificate:
MITHALI MOHTA O ( this is the actual arrangement in my certificate)
Given Name: MITHALI
Surname: MOHTA
Initial: O ( O stands for my dad's name first letter and it is my Middle Name)

In my Passport: MITHALI OMPRAKASH MOHTA (the initial O is expanded here and its placement is in the middle)
Given Name: MITHALI
SURNAME : OMPRAKASH MOHTA

Do I need to arrange for any supporting documents? And how should I mention my name in wes application?

I don't think it might cause you any trouble. In my case, I got my name printed in various orders on my diplomas and documents due to the cultural difference between Eastern and Western countries and my files have been completely good to go. For example:
-Name on PCC from Vietnam and passport: Tran Hoang Truong Hai.
-Name on US degree: Hai Tran.
-Name on IRCC eAPR: Hoang Truong Hai Tran.
