Name difference in Academic certificates and Passport for WES evaluation

Dear sirs,

I am applying for WES evaluation . My name in Academic certificate is DIJO M VARGHESE , where as in my passport is DIJO MATHEW VARGHESE. Please let me know what are the documents required for supporting my expansion of initial. Also let me know by which name I have to create WES profile.
