name conflict between passport & Birth cetificate

hi guys,

please advise regarding name conflict between passport & Birth cetificate (india) of my 2 year old daughter

on passport : Dhanyaa Balasubramani

on Birth certificate : S.B.Dhanyaa (s-wife name, B-my name, Dhanyaa-daughters name)

currently on application form IMM0008 generic

1.under personal detail for full name - i had used her name as in passport

2.under have you ever used any other names - i have selected "yes", than for family name i wrote "wife name + my name", for given name "Dhanyaa"

Guys is it ok or do i have to go for any other option like notarize...etc

I would STRONGLY recommend an Affidavit explaining everything. Thats what I have done. you should be okay.

Your city Corporation will make amends to birth certificate if you approach them with all relevant records. This is by far the best method. In future too, you may possibly have challenges explaining the difference in names. Suggest you eliminate the issue, from long term perspective. Just my thought.
