Hello everyone,

My name is BHARGAV BHARATHAN. When I was born, my parents registered my name as R.B. BHARGAV in the birth certificate. Later on in my first passport they gave my name as BHARATHAN BHARGAV which went on to become my official name.

I changed my name in the government gazette from BHARATHAN BHARGAV to BHARGAV BHARATHAN and have the proof for it.

I also got a notarized affidavit claiming the person R.B. BHARGAV in my birth certificate is infact BHARGAV BHARATHAN.

My question is, in my application should I mention both the incorrect names? ie. R.B. BHARGAV under birth and BHARGAV BHARATHAN under former name. I can then provide the necessary documents to support the name change and a letter of explanation too.

Yes, under the field for alternate names/aliases in the questionnaire. Also proceed with the documents and LOE.
