Name change document

My wife had a different birth name
So, now she has gone through two name change
1)birth name
2)Changed new name
3)Middle name changer after marriage (Passport name)

There are two documents from Government of india
changing from 1 to 2
changing from 1 to 3

Her passport has 1 and 3 name
All other documents including graduation certificate has 2 name.

What should I do in express entry to explain this??

I am unfamiliar with what u can do in express entry but I feel u will need one and the same person affidavit when filling the application after ITA

I have 100 rs stemp affidavit for 1 to 3 name change and I have gazetted document from gov of Maharashtra stating 1 to 2 name change.

Is it valid ?

I will let someone else answer this who is familiar with this..
I personally used one and the same person affidavit stating that all these names belong to one person..
