N/A Forms & Income Question

If a form isn't applicable such as use of a representative do we need to include that form with n/a written on it?

That is what I did and I got approved with no problems.

One more question:

On the Sponsorship Evaluation it asks you 'Your net personal income', my girlfriend (the sponsor) is in the UK on a work holiday visa but has to return home in June. She doesn't have a job and we have given evidence about her intent to return home but does she need an Option C Printout and to fill in the personal income field?
